Since 1972, Central Florida Health Care has helped make quality, affordable care easier to reach for individuals and families across Polk, Highlands, and Hardee Counties.

What began in Frostproof as a response to urgent health care gaps has grown into a regional network committed to accessible, patient-centered care. CFHC meets patients where they are with medical, dental, pharmacy, behavioral health, specialty, and enabling services close to home.
Today, that small beginning has grown into a far-reaching mission of care. CFHC is now one of 1400 Federally Qualified Health Centers (FQHC) in the U.S. With 18 health centers across Polk, Highlands, and Hardee Counties—including a unique Community Partnership School at Crystal Lake Elementary School in Lakeland—CFHC continues to meet people where they are. Mobile healthcare units also extend that mission even farther, bringing essential services to some of the most rural and underserved communities in the tri-county area.
